Keep two issues separate
Judge Conduct and Decision Review Checklist
Use this tool now
Organize existing records about a judge’s conduct and a disputed decision without confusing an ethics complaint with a case filing. This checklist does not calculate deadlines, submit a complaint, or change a decision.
How to use it
- 1.Contact counsel promptly about any decision or upcoming hearing; do not wait to finish this checklist.
- 2.Use copies and preserve original records. If represented, review any planned submission with your lawyer.
- 3.Keep completed notes private. Use the official complaint form and current instructions if a complaint is appropriate.

Judge Conduct and Decision Review Checklist
Preserve the complete decision
Include attachments, proof of service, envelope or electronic delivery record, and actual receipt date.
Identify the conduct event
Locate the hearing notice; record date, district office, judge, case number, and people present.
Separate conduct from legal disagreement
Identify the exact words or actions separately from the ruling you believe is wrong. Do not guess motives.
Locate supporting records
Keep existing transcripts and correspondence; identify relevant pages. Date any later recollection honestly.
Ask about the correct procedure and deadline
Have counsel review the actual order and case posture. An ethics complaint is not reconsideration, removal, or disqualification.
Track submissions and next hearing separately
Save copies, delivery proof, and responses. Do not treat a complaint as changing an order or excusing attendance.
Avoid
- — Do not wait for an ethics response before seeking help with case deadlines.
- — Do not alter records or privately contact the judge to argue the case.
- — Do not post case or medical information publicly.
Keep with this tool

Common questions
Does this checklist file a complaint?
No. It organizes records. Use the official ethics form and current instructions for a complaint, and obtain separate advice about case filings.
Does an unfavorable ruling establish misconduct?
No. A legal disagreement alone is different from specific conduct that may violate an ethical rule.
Will a complaint change the decision?
The official form says an ethics complaint will not reverse or change the judge’s decision. Ask promptly about the applicable case-review procedure.
