Prepare for a treatment visit

Workers' Comp Video Treatment Visit Checklist

Use this tool now

Use this checklist for a treatment appointment by video, not a QME or AME examination. Confirm the arrangements before the visit and preserve what happened if the connection or communication fails.

How to use it

  1. 1.Use one checklist for each treatment video visit. Copy details from the appointment notice.
  2. 2.Contact the clinic before the visit about missing records, language needs, privacy, device access, or connection problems.
  3. 3.Keep this checklist privately with your appointment records; do not put passwords or access codes on it.

Checklist

Workers' Comp Video Treatment Visit Checklist

  • Identify the appointment

Confirm the clinician, date, time, claim arrangement, and whether this is treatment rather than a QME or AME exam.

  • Confirm the format and alternatives

Ask the clinician about consent, limits of video, and whether an in-person examination is needed.

  • Arrange communication support

Request the needed language and confirm how the interpreter or other assistance will join.

  • Check the technology and backup contact

Confirm the platform instructions and clinic number to call if the connection fails.

  • Prepare existing medical records

Ask how to securely provide reports, test results, medication list, and current work-status note.

  • Describe actual work and symptoms

Give accurate examples. Do not recreate the injury or perform unsafe movements for the camera.

  • Document a failed or incomplete visit

Record join attempts, technical errors, contacts, and responses without unnecessary private information.

  • Confirm the next step

Ask whether the visit was completed, whether another format is needed, and when follow-up will occur.

  • Request the written visit record

Ask for the summary, treatment plan, restrictions, and status of any treatment-authorization request.

Avoid

  • — Do not treat this checklist as permission to cancel a QME or AME examination.
  • — Do not assume a video link proves authorization or that one failed call permits any outside doctor.
  • — Do not record the consultation without first addressing permission and applicable rules.

Keep with this tool

  • — Appointment notice
  • — Requests for access or language help and responses
  • — Technical-failure timeline
  • — Visit summary and work-status note
  • — Treatment-request and follow-up records

Common questions

What if I cannot use video?

Tell the clinic early and ask what alternative can be arranged. If an MPN applies, its medical access assistant can help with scheduling and access.

Is a QME video exam the same?

No. Remote medical-legal evaluations have separate conditions, including required written agreement. Ask your lawyer or DWC Information and Assistance about the process.

Does a recommendation mean treatment is approved?

Not necessarily. Ask whether the treating physician submitted a request for authorization and keep the written response.
