Avoid

  • Do not enter a restricted area or disturb debris to create evidence.
  • Do not identify an unknown chemical based only on an odor or symptom.
  • Do not alter, backdate, or overwrite an earlier record.

  • Lawfully obtained photos, site maps, and air-quality notices

Common questions

Should I use one row for an entire day?

Use another row when the location, route, task, conditions, protection, or symptoms changed enough that one entry would be unclear.

What should I write under observed conditions?

Record what you personally saw, smelled, handled, or noticed, such as visible smoke, soot, ash, dust, residue, debris, unusual odors, or poor ventilation.

What if I noticed no symptoms during a shift?

You may write that no symptoms were noticed during that shift and add a later dated row if symptoms appeared or changed.

Does this log show that work caused a medical condition?

No. It organizes observations and records for review; it does not diagnose a condition or decide medical or claim causation.
