Identify the document
Mark whether it is a bill, statement, explanation of benefits (EOB), Explanation of Review, denial, or collection notice.
Record the provider and service details
Save provider name, account number, service dates, itemization, and contact details.
Record the claim details
Save the claim number, administrator, adjuster, status, and verified billing destination.
Identify the treatment basis
Save the referral, authorization, request for authorization (RFA), medical provider network (MPN) notice, emergency record, or other relevant document.
Ask where the bill was submitted
Record the date, recipient, attachments, and proof of transmission.
Ask what response the provider received
Request the payment, contest, denial, incomplete-bill notice, or Explanation of Review.
Respond to direct collection in writing
Identify the work-injury claim, request an itemization and basis, and keep delivery proof.
Record each follow-up and balance
Do not overwrite an earlier amount or explanation.
